How to Request an Eden Prairie Death Certificate
Four request methods are available through Hennepin County. Choose the one that fits your situation.
In Person: Visit the Government Center at 300 S 6th St in Minneapolis. Bring a valid government-issued photo ID. Fill out the request form, pay the fee, and the clerk processes the request. If the record is in the system, same-day processing is typically available.
By Mail: Fill out the county's death certificate request form or write a request letter with the name of the deceased, date and city of death, your name, your relationship, and your contact information. Notarize your signature. Include a photocopy of your photo ID and a check or money order payable to Hennepin County. Mail to the Government Center address. Allow extra time for mail processing.
Fax: Submit the completed request form and a copy of your photo ID by fax. An additional $9.50 service charge applies. The fax option is faster than mail but still takes longer than an in-person visit. Check the Hennepin County website for the fax number.
Drop Off: Leave your paperwork at the Government Center without waiting at the counter. This works if you can make the trip downtown but want to avoid a long in-person wait. Processing begins after the drop-off is received by staff.

Eden Prairie Death Certificate Fees
Minnesota sets the standard fee schedule for death certificates. Hennepin County follows these amounts. Current fees: $13 for the first certified copy, $6 for each additional certified copy ordered at the same time, and $13 for the first non-certified copy.
Fax requests add $9.50 to the total. If you submit a fax request for two certified copies, the total would be $13 plus $6 plus the $9.50 fax fee, or $28.50. Mail and in-person requests do not carry the extra fee.
Veterans and their families may qualify for one free certified copy. You will need to document the military service connection. Who Can Request Eden Prairie Death Records
Certified death certificates are restricted to people with a tangible interest under Minnesota Statute 144.225. Qualifying individuals include the surviving spouse, parent, adult child, sibling, or legal representative such as an estate attorney. Historical Eden Prairie Death Records
